DIRECTIONS FOR APPLICATION FOR LICENSE AS A FOREIGN LEGAL CONSULTANT
(Read all of these directions carefully before attempting to answer any questions.)

1. This is a statement made under oath. Applicant's failure to fully and accurately disclose any fact or information called for by any question may result in the denial of the application, or if applicant shall have been licensed before the discovery thereof, in the revocation of the applicant's license to practice as a foreign legal consultant.

2. Answers must be in BLACK INK, preferably in typewriting, but may be in the applicant's handwriting or hand printing. Write legibly.

3. Unless otherwise indicated, every question and every part of each question must be answered, even though the answer may be negative.

4. Where "YES" or "NO" suffices, employ that form in answer.

5. If the applicant cannot comply with any of the requirements, state in detail the efforts made to comply and why the applicant cannot. Answers on information and belief will be accepted only in connection with matters which the applicant cannot be expected to remember because of extreme youth or other equally sufficient reason. All statements made on information and belief must be so designated in the sworn statement, and the sources of information must be given.

6. Should the space left for any answer be insufficient, the applicant shall complete the answer on a rider page, specifying the number of the question to which it relates. More than one rider may occupy a rider page. Each rider page should be signed.

7. In stating addresses, give street and number, if any, city, town, zip code, county, and state, or foreign country.

8. In giving a period which is continuing as of the date of verification of this sworn statement, state such period as continuing "to date."

IMPORTANT INFORMATION AND INSTRUCTIONS FOR APPLICANT UNDER RULE 3:05
Foreign Legal Consultants
(Please read and be familiar with the following)

1. Forms for application for application for licensure as a foreign legal consultant may be obtained at the office of the Clerk of the Supreme Judicial Court for the County of Suffolk, or at the office of the Board of Bar Examiners or by written request to the Board of Bar Examiners including a 9 x 12 envelope self?addressed with postage for 4 ounces.

2. Applications for licensure as a foreign legal consultant with all supporting documentation shall be filed IN DUPLICATE (copies accepted) in all cases with the Clerk of the Supreme Judicial Court for the County of Suffolk, John Adams Court House, 3 Pemberton Square, Ste. 1300, Boston, MA 02108, with fee (certified bank check or money order) payable to Commonwealth of Massachusetts.

3. The fee for the application is $510.00 and must be paid at the time of filing an application by certified bank check or money order only. Personal checks are not accepted and THE FEE IS NOT REFUNDABLE.

4. Application must include the following, pursuant to Rule 3:05:

  1. a certificate from the professional body or public authority in such foreign country having final jurisdiction over profession discipline, certifying as to the applicant's admission to practice and the date thereof, and as to his or her good standing as such attorney or counselor at law or the equivalent;

  2. a letter of recommendation from one of the members of the executive body of such professional body or public authority or from one of the judges of the highest law court or court of original jurisdiction of such foreign country;

  3. a duly authenticated English translation of such certificate and such letter if, in either case, it is not in English;

  4. affidavits as to the applicant's good moral character and fitness from three reputable persons residing in this Commonwealth and not related to the applicant, one of whom shall be a member of the bar of the Commonwealth; and

  5. such other evidence as to the nature and extent of the applicant's educational and professional qualifications, good moral character and general fitness, and compliance with the requirements of Section 1 of this Rule as the Board of Bar Examiners may require.

MAIL OR HAND-DELIVER YOUR APPLICATION TO THE FOLLOWING: Supreme Judicial Court Clerk's Office for Suffolk County John Adams Courthouse, 3 Pemberton Sq., Ste. 1300, Boston, MA 02108 APPLICATIONS NOT FILED IN DUPLICATE WILL NOT BE ACCEPTED AND WILL BE RETURNED TO THE APPLICANT.
