1. You must submit the completed
application for Certificate of admission and good standing
with the required $15.00 fee to this office.
The application for the Certificate may be
downloaded or requested by mail, fax or by
E-mail.
2.Upon receipt of the application and the fee, this office will process the request within 3 to 5 business days and the Certificate of Admission and Good Standing will be mailed to you.
Please be advised that the signed
application is valid for a period of 30 days.
Please note if your application discloses an
informal admonition or private reprimand, you should request that the
application be placed in a non-public file.
