1. You must submit the completed
application for Certificate of admission and good standing
with the required $15.00 fee to this office.
The application for the Certificate may be
downloaded or requested by mail, fax or by
E-mail.
2.Upon receipt of the application and the fee, this office will process the request and the Certificate of Admission and Good Standing will be mailed to you.
Please be advised that the signed
application is valid for a period of 30 days.
Please note if your application discloses an
informal admonition or private reprimand, you should request that the
application be placed in a non-public file.
